Alternatively, you can use the following registration form (mirror site here) to pay by check, money order, or credit card. Once you have completed the form, please send it to the address listed on the form. You can also email it as an attachment to the email address listed on the form.

Registration Instructions

To ensure that the correct attendee gets registered for the conference please register one attendee at a time.

Paying by Credit card via Paypal?

  • If paying by personal credit card you may securely enter your Billing Information in the initial request.  
    • Billing Information will be sent to Paypal as the name & address on the credit card.
  • If paying using a corporate credit card enter the attendees name in the initial Billing information
    • Change it to the correct "Billing information" once you are actually paying at Paypal.
    • This assures that the correct person is registered for the conference.
